INITIAL DIAGNOSTIC ASSESSMENT:

usually completed in three steps:

Step 1 (you):

complete the Questionnaire, at home, in your own time. I will email you a personalized copy of the Questionnaire after our initial conversation.

Step 2 (me):

I will assess your initial responses to the Questionnaire.

Step 3 (us):

face-to-face appointments (video or in the office).

FOLLOW-UP APPOINTMENTS: see below

If your appointment is in my physical OFFICE please arrive to 245 Fifth Avenue Floor 3, New York, NY 10016, a few minutes before your scheduled time. The building concierge might require you to show an ID in order to enter the building. The third-floor reception desk personnel will assist you or you may check in at the computer located at the reception. It will send me a text announcing you and I will come out to take you to my office.

Before all ONLINE appointments, please ensure/recheck that you have: access to a device with a screen, microphone, speakers, and a high-quality internet connection. Please be in a quiet, private room.

Initial Diagnostic Assessment – $1000

On average, it will require three or more separate, 45 min appointments. NOTE: regardless of the number of appointments the fee is the same.

Follow Up Psychiatric Appointment – $400 (30 to 50 minutes)

Psychotherapy Appointment - $400 (45 minutes)

Informational Phone Call – FREE

I will provide you with bills in electronic format or paper format, or both. I accept payment via checks and Venmo.

I do not participate in health insurance plans directly, including Medicaid and Medicare. I work as an out-of-network provider.

When this is available, working independently from insurance allows you to choose exactly how your treatment should be structured. It also provides you with more privacy. If you do have health insurance with out-of-network benefits, I will provide you with detailed bills for all your appointments. You can then send them to your health insurance provider for direct reimbursement to you.

While this can vary widely between plans, the reimbursement is commonly at 50% to 80% of the local psychiatrist fees. You can also select to have Reimbursify.com service do it for you for a small cost.

I understand that health insurance rules can be confusing. I provide a short handout with written instructions on what exactly to ask your health insurance representative and determine your exact reimbursement benefits. Please request a copy and I can email it to you.
